Allegany County Health Department – Komen Kares Breast Cancer Support Services, Western NY Region
Provides transportation assistance and medical assistance for out-of-pocket expenses, including co-pays and deductibles, for Allegany and Cattaraugus County residents diagnosed with breast cancer.
Arnot Ogden Medical Center – Reducing Financial Barriers to Treatment, Twin Tiers Region
Provides transportation assistance and medical assistance for out-of-pocket expenses, including co-pays and deductibles, for Chemung and Steuben County residents diagnosed with breast cancer.
Cancer Resource Center of the Finger Lakes – Breast Cancer Survivorship Initiative, Twin Tiers Region
Provides support to women who have completed breast cancer treatment by providing one-to-one navigation and on-going support groups in Tompkins County.
Canton-Potsdam Hospital – Breast Cancer Patient Navigation Program, Central NY Region
Provides Patient Navigation to reduce or eliminate the barriers to screening, diagnosis, and treatment; and to provide people with education regarding the importance of breast health and early detection in St. Lawrence County.
Charles Cole Memorial Hospital – Reducing Screening Barriers for Women in Potter & Tioga Counties, Twin Tiers Region
Provides screening and diagnostic mammograms to under/un- insured women in Potter and Tioga counties.
Crouse Health Foundation – Breast Cancer Awareness Outreach, Education and Screening, Central NY Region
Provides outreach, education and screening to enhance awareness of breast cancer to underserved women in the city of Syracuse, Onondaga county and contiguous rural areas.
ECMC Lifeline Foundation – Breast Cancer Patient Navigation Program, Western NY Region
Provides culturally competent, evidence-based breast health education and screening services to increase the mammography rate among underserved women throughout Allegany, Cattaraugus, Erie and Wyoming counties utilizing the Mobile Mammography Coach.
Erie County Health Department – Erie County Cancer Services Program, Western NY Region
Identifies and overcomes barriers to increase breast cancer screening, provide access to diagnostics and quality care in priority underserved populations in Erie County through outreach, education and individualized navigation.
International Institute of Buffalo’s – Immigrant and Refugee Breast Health Awareness Project, Western NY Region
Addresses the cultural and linguistic barriers faced by the immigrant population in Erie County through the use of interpreters, culturally competent health educators, and attention to cultural barriers in the patient navigation process.
Onondaga County Health Department – Breast Cancer Outreach, Education, and Screening to At-Risk Populations, Central NY Region
Provides breast cancer screening & diagnostic services to underserved and uninsured women who reside in Onondaga County and the City of Syracuse. Increasing awareness among minority women living in the City of Syracuse & Native American women of the Onondaga Nation for free & reduced-cost breast cancer screening services.
Soldiers & Sailors Memorial Hospital – Tioga County PA Mammography Screening Outreach to the Un/Under Insured, Twin Tiers Region
Provides free mammographic screenings with interpretation to the community of Wellsboro and other surrounding communities within Tioga County.
St. Mary’s Healthcare – Addressing Barriers to Breast Cancer Screening and Treatment, Northeastern NY Region
Provides transportation to weekly all-inclusive breast screenings that feature screenings and same-day follow-up diagnostics, and referral to a breast nurse navigator for long-term case management in Fulton and Montgomery counties.
The Fund for Women – FFW Barriers to Care, Twin Tiers Region
Provides financial assistance to women in need for support of their breast cancer treatment in Chemung, Schuyler and Steuben counties.
The Guthrie Clinic – Breast Cancer: Screening for Life, Twin Tiers Region
Provides an evidence-based, culturally relevant breast cancer education opportunity in a group setting addressing educational and screening recommendation needs for Tioga County, NY residents.
The Upstate Hospital Foundation – She Matters – Increasing Screening Mammography in Underserved Women, Central NY Region
Provides peer outreach, education, navigation, and screening mammography to reduce breast cancer disparities in low-income, primarily African American women in the City of Syracuse.
To Life – Education and Support for Underrepresented Minorities, Northeastern NY region
Provides the services of a bi-lingual breast health educator to help remove barriers (language and transportation) to diagnostics and treatment for women in Schenectady, Fulton and Montgomery Counties. Offers a monthly support group for individuals living with metastatic breast cancer
University at Buffalo Research Foundation’s – Patient Ambassadors to Activate Breast Health Continuum Care in Underserved Primary Care Practices, Western NY Region
Provides screening events at primary care practices on the East Side and West Side of Buffalo. Uses trained Patient Ambassadors to provide breast health education and assist women through the screening steps.
University of Rochester – UR Medicine Breast Imaging Outreach, Central NY Region
Provides Screening and Diagnostic support for un/under-insured patients, transportation to and from appointments, patient navigation, and outreach/education in Monroe County.
Whitney M. Young, Jr. Health Center, Inc. – Breast Health Project, Northeastern NY Region
Addresses the need for increased access to breast cancer screening among African American and foreign born, low-income women in Albany County.
YWCA of Binghamton/Broome County – ENCOREplus Breast Cancer Outreach, Education & Advocacy, Twin Tiers Region
Provides breast health outreach, education and advocacy services in Broome & Tioga counties. Provides case management services to help women overcome barriers to care, including financial, transportation and child/eldercare.
YWCA Elmira & Twin Tiers – YWCA Elmira Women’s Breast Health Program, Twin Tiers Region
Provides Education and Outreach, a Patient Relief Fund, and services of the Pink Boutique to residents throughout the Twin Tiers Region.
YWCA of Northeastern New York – Encore: Take Charge of Your Health Program, Northeastern NY Region
Provides breast health education to women in Schenectady and Montgomery Counties by offering educational sessions and one-on-one discussions.
